The "Community Choice" Program Counselors work with seniors, caregivers and families to:

-Explain any financial or medical requirements
-Coordinate with the nursing care facility discharge planner to find services which support senior's dignity, choice and independence
-Help make arrangements so that seniors are housed in a happy, safe and appropriate community setting
