SERVICE PAYMENTS FOR THE ELDERLY AND DISABLED

Program Description

The Service Payments for the Elderly and Disabled Program provides free in-home care support services for older adults, the physically disabled or those with mobility issues.

Free In-Home Care Support Services for elderly and others include:

  • Help with Chores and Yard Work - snow removal, yard work and heavy cleaning.
  • Emergency Response System (Lifeline) - provides telephone emergency response.
  • Home Modifications- home safety modification to allow for independent living such as installation of grab bars or safety rails.
  • Family Home Care – reimbursement to a family caregiver defined by state law and resides in a client's home 24-hours per day.
  • Homemaker cleaning services - provides house cleaning, laundry, and/or meal preparation services.
  • Personal Care – assistance with bathing, dressing, transferring, toileting, and supervision.
  • Respite Care - temporary relief to full-time caregiver.
  • Adult Family Foster Care - safe, supervised family living environment, 24-hours per day in a state licensed setting
  • Case Management - care planning, referrals, and service monitoring.
